WHAT IS INTAKE?
Rock County Behavioral Health Intake serves as the “front door” for HSD behavioral health services and provides welcoming screening, information, and access to human services programs. Intake staff are trained counselors that can help clients, family members, community members, and referring providers link to the best behavioral health service to meet their needs. These services may include referral to community agencies, assistance with understanding services available for insured individuals, as well as screening and assessment for admissions to Rock County Behavioral Health treatment services and programs.

WHAT TREATMENT AND SERVICES ARE AVAILABLE?

  • Phone screening for behavioral health needs
  • Behavioral health comprehensive assessments
  • Information and referral for Community Support Programs (CSP)
  • Information and referral for Comprehensive Community Services (CCS)

WHO IS ELIGIBLE?
Eligibility for services varies by program. We strive to primarily serve individuals who are not able to access appropriate services in private community agencies. Some programs have state-mandated criteria for admission, and intake workers can assist with referral for the appropriate service based on eligibility. Please note that some programs may also have wait lists for admissions.

HOW DO CLIENTS PAY FOR SERVICES?
There are no fees for phone screening, information, or referral services. Behavioral health assessments will be billed to applicable insurance or billed to the individual based on a sliding fee scale. Individuals should bring proof of insurance and income verification to the initial assessment appointment.
