WHAT IS CLTS?
CLTS is a Medicaid Waiver Program designed to serve children with long term support needs. CLTS provides funding for services to support children with physical disabilities, developmental disabilities or severe emotional disturbances who would otherwise be at risk for institutional level care. Funding can be used to support a range of services based on an assessment of the needs of the child and his or her family.

WHAT TREATMENT AND SERVICES ARE AVAILABLE?
There are a broad range of services offered through the CLTS Waiver Program to meet the needs and goals of your child and your family. These services may help your child perform activities of daily living, participate in home and community activities, control their environment and have enriched surroundings. Some examples of possible services include:

  • Adaptive aids
  • Service coordination
  • Communication aids
  • In-home treatment therapy
  • Daily living skills training
  • Day services
  • Respite
  • Home modifications
  • Supportive home care

WHO IS ELIGIBLE?
The CLTS Waivers are available to children and youth under the age of 22 who have a developmental disability, physical disability or severe emotional disturbance.

HOW DO CLIENTS PAY FOR SERVICES?
Services are available through the CLTS Medicaid Waiver Program. Clients must be Medicaid eligible at the time of enrollment. A parental fee based on the family’s income may apply.
